Keep track of your treatments using this form. Treatment Session Records are required for advancement to the next level seminar.
Use the Treatment Session Record to document the recipient’s name, date of the session, length of treatment, and the hibiki (sensations) experienced during the session. Additional copies may be obtained as needed.
Treatment Session Record forms are provided at no charge. Select “Add to Bag” and complete checkout by entering your name, email address, and state. A link to download the form will be sent to the email address provided.
