Use the Shinpiden Seminar Registration Form to collect participant information and maintain a record of their enrollment and participation in the seminar. Additional copies may be obtained as needed.
Seminar Registration Forms are provided at no charge. Select “Add to Bag” and complete checkout by entering your name, email address, and state. A link to download the forms will be sent to the email address provided.
The download link will remain active for 30 days.
